		Couple Life Member Fee			₹ 25,000

APPLICATION REQUIREMENTS -

(For Fresh applicants who want to become IMA member)

1. Form - Duly filled and signed form of 4 pages

Passport Size Photograph on every page of form (Photographs needed - 4)

2. Photocopy of MCI Registration & Degree

3. Fee (Payment through cheques only)

Single Member - Fee Rs 18000/- , But Cheque must be given in two part

1. One cheque of Rs. 3,000/-in favour of “Allahabad Medical Association”

2. Second cheque of Rs. 15,000/-in favour of "IMA UP State"

Couple Member - Fee Rs 25,000/-, But Cheque must be given in two parts

1. One of Rs. 5,000/-in favor of “Allahabad Medical Association”

2. Another of Rs. 20,000/-in favor of "IMA UP State"

Women's Wing MEMBERSHIP APPLICATION REQUIREMENTS :

1. Form - Dully filled and signed form of 1 page
